About the role

We're hiring a part-time Referral Marketing Representative to serve as the face of EHC to Skilled Nursing Facilities (SNFs), rehab centers, Assisted Living Facilities (ALFs), and Memory Care communities throughout the Omaha area. This is a relationship and business-development role — not clinical care — focused on building trust with discharge planners, social workers, and facility staff so that EHC becomes their go-to referral for safe, high-quality in-home care.

This role is ideal for someone who has walked in the shoes of a discharge planner or medical social worker and understands firsthand how referral decisions get made, what facility staff need to feel confident making a referral, and how to navigate those relationships with professionalism and empathy.



Responsibilities

  • Build and maintain relationships with discharge planners, social workers, and case managers at SNFs, rehab facilities, ALFs, and memory care communities across Omaha
  • Conduct regular in-person visits and drop-ins across an assigned territory/route
  • Present EHC's value proposition — including Sensi Co-Pilot and GUIDE Program participation — to facility staff and decision-makers
  • Deliver marketing materials, respond to facility questions, and act as a trusted point of contact for referral partners
  • Track outreach activity, follow up consistently, and report on referral pipeline progress
  • Identify new partnership opportunities and help expand EHC's referral network
  • Collaborate with EHC leadership (Steve & Jerri) on messaging, targeting, and territory strategy
  • Represent EHC at community events, lunch-and-learns, and in-service presentations as needed


Qualifications

  • Experience as a social worker, discharge planner, or case manager strongly preferred — you understand the referral process from the inside
  • Existing relationships or familiarity with the Omaha-area SNF/rehab/ALF/memory care landscape a major plus
  • Comfortable and confident with face-to-face relationship building and in-person outreach
  • Self-motivated — able to manage your own schedule and territory with minimal oversight
  • Strong communicator who can build trust quickly and follow through consistently
  • Reliable transportation for travel throughout the Omaha metro
  • Genuine care for seniors and families, and alignment with EHC's mission-driven, relationship-first approach
 

Schedule & Compensation

  • Part-time, flexible schedule built around facility visit hours
  • Base pay plus commission/bonus structure tied to qualified referrals and conversions (details discussed during interview)
  • Mileage reimbursement for local travel
