About the Role
Executive Home Care of Omaha is looking for a motivated, personable, and relationship-focused Referral Marketing Representative to help grow our referral network throughout the Omaha metro area.
This is a community-based relationship-building and business development position — not a clinical care role. You will build and maintain professional relationships with Skilled Nursing Facilities (SNFs), rehabilitation centers, Assisted Living Facilities (ALFs), Memory Care communities, hospitals, and other senior care organizations.
You will connect with discharge planners, social workers, case managers, and facility professionals who help seniors and their families transition from facility-based care to dependable in-home care.
This opportunity is especially well suited for someone with a background in social work, discharge planning, case management, healthcare, or senior care who understands how referral relationships are developed and enjoys making connections in the community.
What You'll Do
- Build and maintain strong professional relationships with discharge planners, social workers, case managers, and healthcare professionals
- Develop referral partnerships with SNFs, rehabilitation facilities, ALFs, Memory Care communities, hospitals, and other senior care organizations
- Conduct regular in-person facility visits, introductions, and follow-ups
- Present Executive Home Care's services and value proposition to referral partners
- Educate healthcare professionals and facility partners about EHC programs, including Sensi Co-Pilot and the GUIDE Program
- Serve as a reliable point of contact for referral sources and respond to questions or requests
- Deliver marketing materials and maintain consistent communication with referral partners
- Identify new referral and business development opportunities throughout the Omaha metro
- Track outreach activities, follow-ups, referrals, and relationship development
- Represent Executive Home Care at community events, lunch-and-learns, networking events, and in-service presentations
- Collaborate directly with EHC leadership on outreach strategy, messaging, and territory development
- Help grow and strengthen Executive Home Care's referral network and community presence
Qualifications

5+ years of experience in social work, discharge planning, case management, healthcare, senior care, or a related field
- Experience working with hospitals, SNFs, rehabilitation facilities, Assisted Living, Memory Care, or other senior care settings strongly preferred
- Understanding of the healthcare referral, care transition, and discharge planning process
- Familiarity with the Omaha healthcare and senior care community is a strong plus
- Comfortable with face-to-face networking, facility visits, presentations, and professional outreach
- Excellent communication, interpersonal, and relationship-building skills
- Professional, personable, and confident when interacting with healthcare professionals
- Self-motivated and able to manage your own schedule, territory, and follow-up activities
- Strong organizational, time-management, and follow-through skills
- Reliable transportation and ability to travel throughout the Omaha metro area
- Genuine interest in helping seniors and families access quality in-home care
Schedule & Compensation
- Part-time position
- Flexible Monday–Friday schedule
- Schedule structured around facility visits, networking, and referral-partner availability
- Base pay + commission/bonus opportunity
- Commission/bonus structure tied to qualified referrals and conversions; details discussed during the interview
