Care Coordination With Purpose
We’re hiring a Client Care Coordinator to play a critical role in managing and evolving client care beyond the four walls of the home. This position sits just below a Care Manager and is ideal for someone who understands how care, insurance, families, and multiple health partners all intersect.
What You’ll Do
- Coordinate and actively manage individualized care plans as client needs change
- Serve as a trusted point of contact for clients and families
- Navigate insurance basics, community resources, and care options
- Collaborate with physicians’ offices, hospitals, PACE programs, and other care partners
- Identify gaps or changes in care needs and bring recommendations to leadership
- Support care transitions and ensure continuity across settings
What We’re Looking For
- Experience in care coordination, healthcare administration, home care, case management support, or social services
- Comfortable communicating with families and multiple healthcare stakeholders
- Strong organizational skills and sound judgment
- Clinical license not required, but healthcare experience preferred (Home Care, Social Work, Home Health Care, etc)
- Ability to complete background screening through the Florida Care Provider Background Screening Clearinghouse
