The Opportunity

We are seeking a dedicated RN Assessment Specialist to partner with Executive Home Care of Wake Forest as a contractor. This long-term partnership opportunity offers independence with stability and growth potential. As our Assessment Specialist, you'll conduct comprehensive non-medical evaluations and develop personalized care plans that become the foundation for exceptional service delivery.

Work primarily from your preferred location in this remote role. On-site presence is required only for compliance activities, caregiver education sessions or skill development, and quality assurance reviews, etc.

Key Responsibilities

  • Conduct comprehensive non-medical needs assessments using our structured assessment packages
  • Develop and document detailed, personalized non-medical care plans tailored to each client's unique needs
  • Perform regularly scheduled quality assurance visits of client care
  • Generate leads and/or assist with converting leads to clients; while participating in business development activities to grow the client base to achieve monthly client conversion goals
  • Provide care coordination services between clients, families, and their existing healthcare providers
  • Be available for telephone consultation during agreed parameters.
  • Provide caregiver education sessions or skill development
Qualifications

  • Current, unrestricted RN license in North Carolina
  • Minimum of 2 years of nursing experience (home care experience a plus, not required)
  • Passion for supporting seniors and adults with diverse care needs
  • Strong assessment and documentation skills
  • Excellent communication and interpersonal abilities
  • Demonstrated ability to work independently and manage time effectively
  • Reliable transportation to travel throughout the Triangle service area
  • Ability to provide your own tools/equipment
Fee Structure

Compensation is based on our hybrid fee structure, which includes incentives across lead generation, client conversion, assessments, and client visits. Monthly flat-based fee designated for telephone consultations during agreed parameters, hourly rates for care conferences, caregiver education sessions, and other specialized services. This model pays based on completed services and combines the stability of payments with the opportunity to increase your earnings through your productivity and efficiency.
